Contest Rules, Requirements, and Contest Waiver
Read carefully and sign below:
1. I am a member of the Natural Physique Association (NPA) on sale at contest site ($25).
2. I have paid all fees to enter the contest. Entry fees - $100 and Crossovers $50.00
3. I agree to uphold th honor-code concerning sortsmanship and conduct of the NPA. Failure to do so may result
to reprimand, suspension or expulsion, from the association.
4. Trunks, one piece men; two piece (bikini) women, no jewelry applies to bodybuilders only (wedding bands OK).
5. Provide (2) tapes/90 seconds maximum/ CDs or Cassettes ONLY ONE SONG PER TAPE ALLOWED!
6. No vulgar music tapes will be played!
7. I agree to take a polygraph, urine and or sign a sworn oath that will be notarized if presented to me by notary who's
been assigned by the NPA President or his designee or Contest Promoter.
8. NOte: Rounds (1), (2), and (final) will be conducted in the NPA Figure and Bikini competitions.
9. Figure Rounds (1) bikini or two piece swimsuit), Round (2) and (Final) – (one piece suit). (Figure only)
10. Bikini Rounds (1)(2) and (final) or two piece "v" shaped bottom.
Waiver of Claims : In consideration of your acceptance to this entry form and fee, I waive and release all rights and claims
for damages against the following persons and organizations, Si-Flex Physique Club Inc., the Natural Physique Association,
Harry E. Silas,Melissa Minton-Schaffer, Civic Auditorium, and all their sponsors, association, staff, administrators, officials, agents, and employees.
This waiver applies to any injuries I may acquire while traveling to and from the championships at the Contest site. I further
agree to abide by decisions of the officials, NPA Committee and declare that all the information contained with this application
is true to the best of my knowledge, and is correct and complete. I further agree to hold harmless, waive and release all rights
(photos, name and likeness, for media newspaper/TV video magazines), for the NPA Mr./Ms. Tennessee Bodybuilding &
Fitness/Figure/Bikini Championships. I have carefully read all of this information and understand it. My signature below
